This is for international students that have not returned to the university yet ONLY. You will need to fill this once a day.
Go to either of these URL.
¶ Step 2: Login
Click this button:
and login using your NetID
¶ Step 3: Go to the health report menu
click the daily report option -->
¶ Step 4: Fill the report
You may need some help from translation tools at this step.
International students do not have "class" number, just enter 99 or 9999, it does not matter.
